Essential VII

Interprofessional Collaboration for Improving Patient and Population Health Outcomes

"Recognizes that the master’s-prepared nurse, as a member and leader of interprofessional teams, communicates, collaborates, and consults with other health professionals to manage and coordinate care"

(American Association of Colleges of Nursing, 2011, p. 5).

In order to care for patients effectively, healthcare professionals need to be able to work together and communicate effectively.  When I first began this program, I lacked confidence and professional communication skills essential to communicate as one provider to another.  I did not always use the proper scientific terms, my report was often unorganized, and I did not always know how to communicate my assessment findings in a clear way.  At this point, I am able to communicate with healthcare members including mid-levels, physicians, and nursing staff to coordinate care effectively and efficiently.  It is very important to be able to communicate patient cases effectively so that patients get the appropriate treatment and care.  It's also essential that healthcare providers are able to work together in a respectful way to ensure quality care is carried out. Provers are expected to communicate and work together to ensure that patients reach their health goals.

The Geriatric Health Promotion Project is a project performed by myself and three other graduate nursing students.  The four of us collaborated with the Knoxville Senior Center to provide an informed discussion with seniors at the center on fall precautions.  The PowerPoint was used as a visual tool to assist in the discussion.  The discussion was also taken to the classroom setting as an educational tool for our graduate nursing peers.

The SOAP notes below are patient encounters of patients I've managed in my graduate program with preceptor supervision.  These SOAP notes are examples of how I've managed the care of patients across the lifespan with the collaboration of other health professionals including cardiologists, pediatricians, and gastroenterologists.
